Job summary

WECRUIT PTE. LTD. is seeking a Patient Care Officer to plan, coordinate and supervise the care and well-being of residents, ensuring a clean, safe and comfortable environment.

You will organize programmes, conduct resident interviews, and maintain records while collaborating with internal teams and external providers to meet residents' needs. The role emphasizes compassionate care, teamwork, and accurate documentation.

Qualifications

• Diploma or equivalent qualification (minimum).
• Experience in social services, residential care or related settings is advantageous.
• Good communication and interpersonal skills with residents.
• Patient, compassionate, able to work with diverse care needs.
• Able to work independently and as part of a team.
• Good documentation and record-keeping skills.

Responsibilities

• Plan, coordinate and supervise resident care and well-being.
• Organise and implement programmes to enhance residents' quality of life.
• Conduct regular resident interviews and maintain records.
• Monitor upkeep of equipment, facilities and the environment.
• Ensure a clean, safe and comfortable environment for residents.
• Coordinate with internal teams and external providers to address needs.
• Monitor residents' well-being and escalate concerns when necessary.
• Maintain documentation relating to residents' care and activities.
• Support implementation of operational policies, procedures and service standards.
• Perform other duties as assigned by management.
