PERSOL SINGAPORE PTE. LTD. is seeking a dedicated professional to support residents and families through financial assistance applications and care coordination. You will liaise with healthcare institutions, coordinate referrals, and ensure timely submissions for schemes like Medifund and IDAPE.

Responsibilities include maintaining case files, data entry, and handling inquiries while assisting transfers and discharge processes as needed.

Qualifications

Prior experience in social work, social services, case management, or a related field is preferred.
Experience in the healthcare or community care sector is advantageous.

Responsibilities

Collect and assess social and financial information about residents and their families prior to financial assistance applications.
Act as the primary liaison for residents, families, and healthcare institutions to coordinate referrals and process applications.
Ensure accurate and timely submission of various financial assistance schemes, including but not limited to Medifund and IDAPE.
Maintain and update case files, referrals, and other documentation for effective case management and auditing purposes.
Perform data entry and regularly update information to ensure database accuracy.
Handle administrative tasks related to enquiries, referrals, and resident admissions.
Facilitate transfers to other facilities or healthcare institutions and oversee the discharge process of residents.
