Job summary

NHG Health in Singapore seeks a pharmacist to lead medication review and ensure safe, effective drug therapy under supervision. The role focuses on validating orders, addressing potential interactions, and communicating with prescribing physicians.

The incumbent will dispense medications, provide drug information to clinicians, and maintain strict regulatory compliance. Continuous learning and participation in audits are expected, with involvement in hospital initiatives and outreach activities.

Responsibilities

• Review each patient’s medication chart or prescription for appropriateness under the supervision of a pharmacist.
• Contact the prescribing doctor when an intervention is needed.
• Ensure all orders are clear and specific.
• Verify doses are within therapeutic limits.
• Identify potential adverse reactions and drug interactions.
• Fulfill all drug orders accurately and in a timely manner, under supervision.
• Monitor drug levels where applicable and advise dose adjustments.
• Dispense medications and explain indications and administration instructions under supervision.
• Provide drug information to other healthcare professionals under supervision.
• Perform sales and supply of medicinal products under supervision.
• Engage in continuous learning through journals and publications to stay current in the field.
• Attend structured learning sessions and participate in audits as assigned.
• Ensure compliance with departmental, hospital, and external regulatory policies and practices related to pharmacy operations.
• Complete all Singapore Pharmacy Council mandated training requirements, including workplace-based assessments and portfolio documentation.
• Actively support hospital initiatives, including Community Outreach Programmes, Staff Welfare Activities, and Recreational Events
