Job summary

The National Heart Centre Singapore seeks a Pharmacist to provide pharmaceutical care at our outpatient pharmacy, counselling and dispensing medications, and teaching patients, caregivers and healthcare workers about medications. You will help identify and resolve medication-related problems, ensure accurate distribution of pharmaceutical items, and participate in corporate quality improvement projects.

Senior level applicants with relevant hospital experience are welcome; on-the-job training

Qualifications

• Degree in Pharmacy or equivalent, recognised by the Singapore Pharmacy Board for registration.
• Relevant hospital experience is advantageous but not essential as on-the-job training will be provided.
• Candidates with experience will be appointed at a senior level.

Responsibilities

• Counselling and dispensing of medication to patients.
• Teaching and providing information on medications to patients, caregivers, and other healthcare workers.
• Assist patients with administration of medications and identify, resolve, and prevent medication-related problems.
• Participate in corporate quality improvement projects.

Pharmacist

You will be responsible for the provision of pharmaceutical care at our outpatient pharmacy. Your duties include counselling and dispensing of medication, teaching and providing information on medication to patients,their caregivers and other healthcare workers as well as assist them in the administration of medication. You will also need to help identify, resolve and prevent medication-related problems and ensure accurate and timely distribution of pharmaceutical items. In addition, you will be required to participate in corporate quality improvement projects.

Requirements:

• Degree in Pharmacy or equivalent, recognised by the Singapore Pharmacy Board for registration

• Relevant experience in a hospital setting environment will be advantageous but not essential as on-the-job training will be provided

• Candidates with experience will be appointed at a senior level

Documented proof of:

(i)Measles (or MMR - Mumps, Measles and Rubella) vaccination or serological evidence of measles immunity; and

(ii)Tdap (Tetanus, Diphtheria and Pertussis) vaccination received within the last 10 years, as required for all healthcare workers under the Healthcare Services Act.
