Job summary

RECRUIT EXPRESS PTE LTD in Singapore is seeking a Patient Service Associate to support pre-admission screening and home-based care coordination. You will work with nurses to monitor patients, update care plans, and liaise with caregivers to ensure smooth service delivery.

The role involves documenting progress, escalating concerns to clinicians, and providing basic education to patients while supporting telehealth activities and community referrals. Office-based, 5 days a week.

Qualifications

• Diploma in healthcare field is required.
• At least 1 year patient service/healthcare experience required.
• Comfortable talking to patients by phone or in person.

Responsibilities

• Perform pre-admission screening for patients and caregivers.
• Collaborate with nurses to provide holistic home care and monitor patients.
• Identify deviations from care plans and report to team leads/clinicians.
• Handle enquiries and provide regular updates on care plans.
• Participate in interdisciplinary meetings and input on care plans.
• Document patient care and progress accurately.
• Coordinate with community resources for continuity of care and referrals.
• Develop care plans based on assessed needs.
• Monitor vital signs and escalate red flags to nurses/physicians.
• Provide basic patient education and telehealth monitoring.
• Support lifestyle coaching on diet, nutrition and exercise.
• Ensure necessary equipment and supplies are available; coordinate with vendors.
• Contribute to quality improvement and risk management activities.
• Perform any other ad-hoc duties as required.

Job Scope:

• Performs pre-admission screening for patients and caregivers.
• Works in collaboration with the nurses to manage and provide holistic care to patients in their homes, including monitoring patients' general condition and progress.
• Identifies and reports any deviations from norm, care gaps or issues to appropriate Team Nurse Lead/Nurses/Clinicians.
• Handles enquiries and provide regular updates on care plans with patients and caregivers.
• Participates in interdisciplinary team meetings and provides input on patient care plans.
• Documents patient care and maintain accurate records of patient progress and treatment plans.
• Collaborates with community resources and other healthcare providers to ensure continuity of care and appropriate referrals for relevant community services.
• Develops care plans based on assessed care needs.
• Monitors patient's vital signs and escalates red flags to the respective team nurses/physicians.
• Provides basic patient education.
• Adherence to telehealth monitoring using Telehealth system.
• Lifestyle changes/health coaching eg, diet, nutrition, basic exercise.
• Ensures all necessary equipment and supplies are available for patient care, and coordinates with vendors as needed.
• Participates actively in continuous quality improvement activities, risk management strategies and projects.
• Performs any others ad-hoc duties and responsibilities that may be assigned from time to time.

Job Requirements:

• Min. Diploma in any healthcare relevant field
• Min. 1 yr patient service associate/ healthcare experience required
• Comfortable with talking/ calling patients to assist them
• Knowledge of hospital operations/previous working experience (in healthcare setting) will be an added advantage
