Job description

Responsibilities for Palliative Care Nurse:
  • Provide safe, competent, and individualised nursing care while maintaining high standards in documentation, treatment, patient monitoring, and infection control practices.
  • Manage complex patient care by applying clinical knowledge, critical thinking, and nursing skills to achieve optimal patient outcomes.
  • Assess, monitor, and evaluate patients’ conditions, develop appropriate care plans, and support patients and families in managing advanced illnesses.
  • Coordinate patient care with physicians and multidisciplinary teams while acting as a liaison between patients, families, and healthcare providers.
  • Build strong relationships with patients and caregivers, assess home care needs, and recommend appropriate training, equipment, and support services.
  • Ensure continuity of care by following up on treatment plans and coordinating necessary resources.
  • Perform nursing procedures including medication administration, wound care, and other clinical interventions as required.
  • Maintain accurate and timely documentation of patient conditions, referrals, enquiries, and attendance records.
  • Participate in multidisciplinary team meetings and collaborate with healthcare professionals to ensure seamless patient care delivery.
  • Promote patient safety by adhering to clinical guidelines, infection control practices, and organisational policies.
  • Ensure proper storage and management of medications, medical supplies, equipment, and consumables in compliance with regulations.
  • Maintain patient confidentiality and handle sensitive information with professionalism and discretion.
  • Provide professional support and respond appropriately to enquiries from patients, families, and the public.
  • Comply with Singapore Nursing Board standards, professional ethics, organisational policies, and regulatory requirements.
  • Stay updated with industry guidelines and continuously develop clinical knowledge and skills.
  • Maintain a clean, organised, and safe working environment.
Qualifications for Palliative Care Nurse:
  • Diploma in Nursing from a recognised local Polytechnic and registered with the Singapore Nursing Board (SNB).
  • Post-registration nursing experience in a hospital, hospice, or community care setting is preferred.
  • Proficient in basic IT systems and electronic documentation platforms.
  • Passionate about delivering quality patient care and providing excellent service to patients, families, and stakeholders.
  • Detail-oriented with strong organisational and time management skills.
  • Good interpersonal and communication skills with the ability to work effectively as part of a multidisciplinary team.
  • Able to work well under pressure and adapt to a fast-paced healthcare environment.
