Job summary

PEOPLE PROFILERS PTE. LTD. is seeking a Medical Social Worker to join a 12-week community reintegration programme.

You will lead home visits with Occupational Therapists, perform psychosocial assessments, identify client goals, and anchor the journey toward community participation. Collaborating in a multidisciplinary team, you will develop client-centered care plans, conduct reviews, support caregivers, facilitate social connections, discharge planning, and follow-ups, while coordinating with

Qualifications

• Degree in Social Work or a Graduate Diploma in Social Work.
• Minimum 2 years of relevant experience in Medical Social Work, preferably in community care, rehabilitation, or home-based intervention programmes.
• Demonstrated experience working within an interdisciplinary team, conducting client/family assessments, and coordinating care with community partners.
• Passionate in supporting clients' functional recovery, community reintegration, and caregiver/family well-being.

Responsibilities

• Lead home visits with OTs during intake and conduct psychosocial assessments to anchor reintegration goals.
• Co-develop client-centered, goal-oriented care plans with OTs aligning functional and psychosocial goals.
• Monitor progress with scheduled reviews and apply motivational interviewing to address barriers.
• Develop Befriender Buddy volunteer scheme and recruit volunteers within subregions.
• Coordinate tasks to RCAs and match clients with Befriender Buddies.
• Introduce social prescription and link to community activities, accompanying first visits.
• Support understanding of the ReAblement process for clients and caregivers, including stress support.
• Facilitate discharge planning and follow-ups with 6- and 12-month touchpoints.
• Document assessments and discharge plans accurately.
• Collaborate with ICCP partners to ensure continuity of care.
