Job summary

PEOPLE PROFILERS (SERVICES) PTE. LTD. is looking for a Social Worker to conduct joint home visits with Occupational Therapists, develop and review client-centred care plans, and coordinate with community partners to support functional recovery and community reintegration.

The role involves case management, motivational interviewing, caregiver support, and oversight of the Befriender Buddy volunteer scheme, with strong emphasis on collaborative, person-centred care.

Key Responsibilities

• Conduct joint home visits with Occupational Therapists (OTs) to assess clients' psychosocial needs, goals, motivations, and barriers to community reintegration.
• Co-develop and review client-centred care plans with OTs, integrating functional, psychosocial, and community participation goals.
• Provide case management, motivational interviewing, caregiver support, and assistance with environmental or home modifications where required.
• Develop and oversee the Befriender Buddy volunteer scheme, including volunteer recruitment, onboarding, training, matching, and ongoing support.
• Coordinate ReAblement Care Associates (RCAs) and Befriender Buddies to support clients' goals and community participation.
• Facilitate social prescribing and linkage to community activities, groups, and services, including accompaniment where needed.
• Support discharge planning, follow-up arrangements, and continuity of care with clients, caregivers, and community partners.
• Facilitate interdisciplinary case discussions with OTs, RCAs, and Befriender Buddies to ensure coordinated, person-centred care.
• Maintain timely documentation and support programme reporting, monitoring, and achievement of programme outcomes.
• Collaborate with ICCP and community partners to strengthen continuity of care beyond the programme.

Qualifications

• Degree in Social Work or Graduate Diploma in Social Work.
• Minimum 2 years of relevant experience, preferably in Medical Social Work, community care, rehabilitation, or home-based services.
• Experience working in interdisciplinary teams and coordinating with community partners.
• Strong interest in supporting functional recovery, community reintegration, and caregiver/family well-being.
• Good communication, engagement, case management, and coordination skills
