Job summary

REACH Community Services Ltd. is seeking a Social Worker (ICCP) to provide clinical social work expertise within the ICCP care model, supporting clients with complex psychosocial and medical needs.

You will work closely with Community Care Executives, hospitals and community partners to develop coordinated, person-centred care plans. Ideal candidates hold a Social Work degree, have at least 2 years of medical social work or case management experience, and familiarity with interRAI and IT

Qualifications

  • Degree in Social Work from a recognised institution.
  • Minimum 2 years of relevant medical social work or case management preferred.
  • Experience with safeguarding matters or crisis situations.
  • Registration/eligibility with SWAAB preferred.
  • Familiarity with interRAI and IT systems (BRIGHT, CCPS, Scribe) is advantageous.
  • Experience with AI-enabled tools is an advantage.

Responsibilities

  • Manage a caseload of clients with complex psychosocial and medical needs.
  • Conduct psychosocial assessments alongside interRAI Check-Up assessments and develop care plans.
  • Coordinate with hospital case management teams, medical social workers and other stakeholders.
  • Provide specialist social work input for safeguarding and high‑risk cases.
  • Contribute to case discussions, supervision and reflective practice.
  • Maintain accurate documentation and support audits and quality improvement.

About the Role

The Social Worker(ICCP) provides clinical social work expertise within the Integrated Community Care Provider (ICCP) care model. The role supports clients with complex psychosocial and medical needs, working closely with Community Care Executives(CCEs), hospitals and community partners to develop coordinated and person-centred care plans.

KeyResponsibilities
Complex Case Management
  • Manage a caseload of clients with complex psychosocial and medical needs, including health, financial, family/caregiving and safeguarding concerns.
  • Conduct psychosocial assessments alongside nterRAI Check-Up assessments and develop appropriate care plans.
  • Coordinate with hospital case management teams, medical social workers and other relevant stakeholders.
Clinical &Practice Support
  • Provide specialist social work input for complex and high-risk cases, including safeguarding concerns, family violence, suicide ideation and financial hardship.
  • Contribute clinical and psychosocial perspectives during case discussions and care planning.
  • Participate in case discussions, supervision and reflective practice, and share knowledge with the ICCP team.
Cross-AgencyCoordination
  • Serve as a key contact for engagement with acute care case management services.
  • Support coordinated care planning involving hospitals, healthcare providers, and community agencies.
Documentation &Practice Standards
  • Contribute to ICCP SOPs, care planning templates and social work practice standards.
  • Maintain accurate and confidential documentation, particularly for complex and safeguarding cases.
  • Support case audits and quality improvement activities.
Requirements
  • Degree in Social Work from a recognised institution.
  • Minimum 2years of relevant experience in medical social work, or in case management roles such as the Community Case Management Service (CCMS) or Community Intervention Team (COMIT, a mental health support service) is preferred.
  • Experience in managing complex psychosocial cases, safeguarding matters or crisis situations.
  • Registration/eligibility with the Social Work Accreditation and Advisory Board (SWAAB) is preferred.
  • Familiarity with interRAI (a standardised care needs assessment tool), and IT systems such as BRIGHT, CCPS, Scribe and AI tools is advantageous.
  • Experience with relevant case management or digital systems and AI-enabled tools is an advantage.
