Job summary

TOUCH COMMUNITY SERVICES LIMITED in Singapore seeks a capable supervisor to lead a team of Care Associates delivering Enhanced Home Personal Care (HPC+) aligned to clients' needs. You will coach staff, coordinate with internal teams, caregivers and external partners to ensure safe, person-centred care and monitor outcomes.

Responsibilities include staff roster planning, performance feedback, documenting care plans, and coordinating resources.

Qualifications

Diploma/Bachelor’s in Nursing, Social Work, Allied Health or related field is required.
Experience in healthcare, eldercare, or community care with supervisory responsibilities is preferred.
Strong leadership, communication and problem-solving skills are essential.
Familiarity with Singapore’s community care sector and regulations is expected.

Responsibilities

Supervise, coach and support a team of Care Associates.
Coordinate with internal teams, caregivers and external partners to deliver HPC+ services.
Monitor client well-being and continuously improve care outcomes.
Plan and manage staff rosters and service coverage during leave or absenteeism.
Ensure compliance with organisational policies, service standards and regulatory requirements.
Maintain accurate service documentation and submit required reports on time.
