Job summary

Care Corner Singapore Ltd is seeking a Social Worker / Social Work Executive (KidSTART Practitioner) to work with parents, caregivers, and relevant systems to support child development, parenting skills, and family well‑being. You will conduct intake, needs identification, and case coordination, while engaging families and maintaining records in SSNet and internal databases.

Qualifications

• Degree or Grad Dip in Social Work or Degree in related disciplines with some experience in social work
• Good understanding of Social Work values and ethics
• Understanding of work with vulnerable populations i.e. low-income families
• Experience in parent engagement work and working with young children is a plus
• Case management experience in social work sector
• Should be comfortable and open to attend occasional night visits and straddle between different locations in East or North.

Responsibilities

• Intake and Enrolment Process referrals from hospitals, community referrals, self-referrals, government agencies and other SSAs
• Engage families about the programme, seek consent from and determine eligibility for KidSTART
• Collate and maintain accurate and complete data in SSNet and on internal databases on referrals and intakes
• Needs Identification and Assessment: conduct timely screening and assessment to support child development and caregiver well-being
• Develop holistic child-centric case plans to coordinate support for identified children
• Case Coordination and Systems Linkages: work with community partners to enhance service delivery and mobilise resources
• Education & Equipping: conduct regular parent engagement sessions and maintain administrative documentation
