• Screen and triage referrals with OT
• Conduct functional screening and InterRAI assessments
• Monitor progress and track milestones
• Coordinate referrals and service delivery for seamless care transitions
• Deliver community integration interventions
• Support caregiver training and transitions
• Conduct home visits with OT or independently for follow ups
• Coordinate meetings with staff and caregivers
• Organise case discussions and progress reviews
• Manage programme documentation and data tracking for reporting
• Support post-programme satisfaction and process surveys
