Job summary

Recruit Express Pte Ltd is seeking an ICU / Palliative Trained Home Care Nurse (Bedok Centre) to deliver compassionate home-based palliative care. The role follows office hours, five days per week, with no on-call on weekdays, and involves visiting patients in their homes to implement care plans.

You will be part of a dedicated team of doctors, nurses and medical social workers, providing holistic care and psycho-social support to patients and families around the clock.

Qualifications

• Experience in home-based nursing care and palliative support.
• Ability to develop and manage care plans for patients.
• Provide psycho-social support to patients and families.

Responsibilities

• Make home visits to provide palliative care and support to terminally ill patients and families.
• Collaborate with doctors, nurses and social workers to coordinate care.
