Job summary

Care for the Elderly Foundation (Singapore) is seeking a dedicated Care Associate to deliver person-centred home-based care for elderly clients. You will support daily living activities, monitor health, and promote independence and dignity within the home environment.

You will work closely with clinical teams to implement care plans, maintain documentation, and ensure safety protocols. Compassion and privacy are essential to this role.

Qualifications

• Experience in home-based elderly care within policies and care plans.
• Ability to work collaboratively with clinical team.
• Commitment to infection prevention and patient safety.

Responsibilities

• Provide personal and daily living care (ADL, hygiene, mobility, feeding) to clients in their homes.
• Monitor clients' general condition and report changes to the clinical team.
• Support psychosocial well-being through companionship and activities.
• Work with nursing staff to implement individualized care plans and participate in home visits as needed.
• Assist with medication-related activities only when trained and authorised.
• Follow infection prevention, PPE, and safety protocols at all times.
• Escalate emergencies and document incidents promptly per procedures.
• Maintain accurate, confidential care records and communicate during handovers.
