Job summary

The Care Associate at Care for the Elderly Foundation (Singapore) provides person-centric care to elderly clients in their homes, promoting independence, safety and dignity while supporting their physical and psychosocial well-being.

You will work with the clinical team to perform ADLs, monitor health, assist with mobility and hygiene, help with meals, and document observations, ensuring privacy, respect and safe, compassionate care at all times, and you should be able to respond calmly to

Responsibilities

• Assist clients with Activities of Daily Living (ADL) according to their assessed needs and care plans.
• Provide assistance with personal hygiene, grooming, toileting, dressing and other personal care needs.
• Assist clients with mobility, transfers, positioning and ambulation using appropriate techniques and equipment.
• Assist clients with feeding and hydration within training and assigned duties.
• Support clients in maintaining independence and functional abilities.
• Maintain a safe, clean and appropriate care environment within the client's home.
• Respect clients' privacy, dignity, cultural preferences and individual choices.
• Monitor and report clients' general condition and changes in physical well-being.
• Assist with basic physiological monitoring (temperature, blood pressure, pulse, blood glucose, oxygen saturation).
• Assist with prescribed mobility, exercise or maintenance activities as instructed by the Clinical team.
• Observe clients during care activities and report deterioration or concerns to the Registered Nurse.
• Follow prescribed care plans and nursing instructions.
• Develop a respectful relationship with clients and caregivers.
• Provide companionship and emotional support within the scope of the role.
• Encourage participation in activities that promote social interaction and well-being.
• Observe changes in mood, behaviour, cognition or emotional well-being.
• Identify and escalate concerns related to neglect or unsafe living conditions per escalation procedures.
• Work collaboratively with Registered Nurses / Nurse Manager.
• Carry out delegated care activities under supervision and training.
• Support implementation of individualised care plans.
• Provide feedback on clients' progress and needs to the care team.
• Assist healthcare professionals during home visits.
• Escalate tasks outside scope of practice or competency.
• Assist with medication-related activities only where trained and authorized.
• Support clients with prescribed topical medication or other delegated care activities.
• Do not perform clinical procedures or administer medication outside approved scope.
• Follow medication safety, infection prevention and clinical protocols.
• Comply with infection prevention and control requirements.
• Practise hand hygiene and use PPE.
• Follow safe work practices when handling, transferring or assisting clients.
• Use care equipment appropriately and report damaged equipment promptly.
• Comply with Workplace Safety and Health requirements.
• Maintain awareness of risks in clients' homes and escalate hazards.
• Respond to emergencies per emergency procedures.
• Maintain First Aid / CPR / AED certification where required.
• Escalate medical emergencies to RN and emergency services when appropriate.
• Report accidents, injuries, falls, abuse concerns and other incidents promptly.
• Complete relevant documentation and incident reports accurately.
• Maintain accurate and timely records of care provided.
• Record observations and changes in clients' condition.
• Communicate information clearly during handovers.
• Maintain confidentiality of client information.
