Job summary

Fresenius Medical Care in Singapore is seeking a Staff Nurse to deliver life-sustaining care for patients with chronic kidney disease. You will work with a multidisciplinary team to provide dialysis treatments, monitor patient progress and offer compassionate support daily.

Requirements include a Diploma/Degree in Nursing, SNB registration, and at least 1 year as a Registered Nurse in Singapore. BCLS certification and HD training are pluses as part of ongoing professional development.

Qualifications

• Diploma or Degree in Nursing.
• Registration with Singapore Nursing Board (SNB).
• Min 1 year as Registered Nurse in Singapore.
• Advanced training in hemodialysis is a plus, renal nurse experience is required.
• Certified and valid Basic Cardiac Life Support (BCLS).

Responsibilities

• Participates in planning, delivery and evaluation of patient care within the hemodialysis clinical setting.
• Undertakes clinical/patient assessments pre, during and post dialysis treatment to evaluate effectiveness of care.
• Reviews blood test results and reports abnormalities to Medical Director.
• Educates patients on diet, fluid, medications and blood pressure management.
• Monitors the water treatment system and fluid for dialysis to ensure safe, quality dialysis therapies.
• Maintains accurate patient information in medical records and TDMS/EuClid systems.
• Acts to protect patient privacy and rights while delivering high standards of care.

You will:

• Participates in the planning, delivery and evaluation of patient care within the hemodialysisclinical setting:
• Undertakes clinical/patient assessments pre, during and post dialysis treatment to evaluateeffectiveness of care
• Adheres to all FMC Policies and Work Instructions related to clinical and operationmanagement
• Reviews blood test results and report to Medical Director of any abnormalities
• Educates patients on their diet, fluid, medications and blood pressure management tooptimise their clinical outcomes
• Motivates and encourages patient’s adjustment to CHD
• Monitors the water treatment system and fluid for dialysis to ensure provision of safe,quality dialysis therapies
• Establish an effective relationship with patients and maintain good communication withpatients and relatives; and give adequate support.
• Ensures/promotes a safe, secure and healthy working environment to staff and acomfortable, safe environment for visitors and patients as detailed in the IPC andenvironment policies
• Ensure all equipment is clean, safe and in good working condition; and report defectsthrough appropriate channels.
• Maintains accurate patient information in medical records, including Therapy DataManagement System (TDMS)/EuClid Clinical Information system and abides by confidentialityand Privacy Policy.
• Protects the rights of patients by adhering to Privacy Principles and providing an excellentstandard of care
• Actively participates in the quality improvement process by reporting all incidents, hazardsand opportunities for improvement
