Qualifications

• Minimum 1 year as Registered Nurse in Singapore.
• Advanced training in hemodialysis is a plus, renal nurse experience is required.
• Certified and valid Basic Cardiac Life Support (BCLS).

Responsibilities

• Participates in planning, delivery and evaluation of patient care within the hemodialysis setting.
• Performs pre/during/post dialysis assessments and evaluates care effectiveness.
• Educates patients on diet, fluids, medications and blood pressure management.
• Maintains accurate patient information in medical records and TDMS/EuClid systems.
• Ensures equipment safety, cleanliness and reports defects promptly.

Job Description

What you'll achieve

As a Staff Nurse, you'll deliver life-sustaining care to patients with chronic kidney disease. You'll work closely with a multidisciplinary team, provide dialysis treatments, monitor patient progress, and offer compassionate support that makes a real difference every single day.

You will:

• Participates in the planning, delivery and evaluation of patient care within the hemodialysis clinical setting:
  • Undertakes clinical/patient assessments pre, during and post dialysis treatment to evaluate effectiveness of care
  • Adheres to all FMC Policies and Work Instructions related to clinical and operation management
  • Reviews blood test results and report to Medical Director of any abnormalities
  • Educates patients on their diet, fluid, medications and blood pressure management to optimise their clinical outcomes
  • Motivates and encourages patient's adjustment to CHD
  • Monitors the water treatment system and fluid for dialysis to ensure provision of safe, quality dialysis therapies
• Establish an effective relationship with patients and maintain good communication with patients and relatives; and give adequate support.
• Ensures/promotes a safe, secure and healthy working environment to staff and a comfortable, safe environment for visitors and patients as detailed in the IPC and environment policies
• Ensure all equipment is clean, safe and in good working condition; and report defects through appropriate channels.
• Maintains accurate patient information in medical records, including Therapy Data Management System (TDMS)/EuClid Clinical Information system and abides by confidentiality and Privacy Policy.
• Protects the rights of patients by adhering to Privacy Principles and providing an excellent standard of care
• Actively participates in the quality improvement process by reporting all incidents, hazards and opportunities for improvement
