Recruit Express Pte Ltd is seeking a Medical Social Worker (MSW) to provide psychosocial, emotional, practical, and spiritual support across the care continuum. You will conduct comprehensive assessments, facilitate care planning, counselling, and crisis intervention, coordinating resources and advocating for residents’ well-being.

You will work with a multidisciplinary team to ensure smooth transitions, uphold PDPA compliant documentation, and support ACP and end-of-life discussions where

Formación

• BA in Social Work or Graduate Diploma in Social Work required.
• 3–5 years’ relevant experience in hospital or community hospital setting.
• Microsoft Office and internet savvy required.
• SWAAB accreditation with Registered Social Worker status required; Advanced Care Plan Facilitator certification preferred.
• Excellent communication and interpersonal skills required.

Responsabilidades

• Provide psychosocial support, counselling, and crisis intervention to residents and families.
• Conduct intake, assessments, and admission support with nursing staff.
• Coordinate case management and referrals to community resources, services, and government agencies.
• Facilitate Advance Care Planning discussions and document preferences.
• Collaborate on discharge planning and transitions across care settings.
• Engage in bereavement support and multidisciplinary collaboration.
