Job summary

SilverConnect.sg is seeking compassionate and experienced Freelance Physiotherapists to provide physiotherapy services to seniors and adults in their homes across Singapore. The successful candidate will conduct assessments, develop treatment plans, provide rehabilitation exercises, and support clients in achieving improved mobility, function, and quality of life.

Responsibilities include home assessments, creating personalised rehabilitation programs, and guidance for carers.

Qualifications

• AHPC-registered physiotherapist with PC in Singapore.
• Degree or Diploma in Physiotherapy from a recognized institution.
• Clinical experience in community care or home therapy settings.
• Able to travel to clients' homes.
• Excellent interpersonal and communication skills.
• Professional indemnity insurance preferred.

Responsibilities

• Conduct home-based physiotherapy assessments.
• Develop individualized treatment and rehabilitation plans.
• Deliver rehabilitation exercises and mobility training.
• Educate clients and caregivers on exercises.
• Maintain treatment records and progress notes.
• Coordinate with doctors and nurses.
• Adhere to professional and ethical standards.

Key Responsibilities

• Conduct physiotherapy assessments in clients' homes
• Develop individualized treatment and rehabilitation plans
• Provide therapy for: Post-stroke rehabilitation, Falls prevention and balance training, Orthopaedic conditions, Neurological conditions, Post-hospitalisation recovery, Geriatric rehabilitation
• Educate clients and caregivers on exercises and mobility techniques
• Maintain accurate treatment records and progress notes
• Communicate with family members, doctors, nurses, and other healthcare professionals where appropriate
• Adhere to professional and ethical standards at all times

Requirements

• Degree or Diploma in Physiotherapy from a recognized institution
• Registered with the Allied Health Professions Council (AHPC)
• Possess a valid and current Practising Certificate (PC) issued by the Allied Health Professions Council (AHPC)
• Relevant clinical experience in community care, rehabilitation, hospital, nursing home, or home therapy settings
• Strong interpersonal and communication skills
• Able to travel to clients' homes
• Professional indemnity insurance preferred
