Job summary

NTUC Health Co-operative Ltd is seeking a Care Navigation team member to deliver customer service with empathy to seniors and families. You will attend calls and emails, assess needs, and guide enrolment into eldercare services, while coordinating with providers and reporting on follow-ups.

Ideal candidates hold a Diploma or degree and have 2–3 years of relevant experience, with patient, tech-savvy, and detail-oriented traits. Training will be provided.

Qualifications

• Be patient and enjoy helping seniors and their families.
• Be a Diploma or degree holder in any discipline.
• Be tech-savvy and able to learn and multitask as various IT systems would be used.
• Have strong communication skills and be meticulous and detail-oriented.

Responsibilities

• Attend customers' calls and email inquiries.
• Assess customers' needs, recommend suitable services, and facilitate enrolment and application processes.
• Conduct needs assessments, including observations and interviews with seniors and their families.
• Collaborate with service providers to ensure timely enrolment and ongoing support.
• Participate in meetings, take minutes, and track follow-up actions.
• Represent NTUC Health at occasional roadshows to promote eldercare services.
• Provide administrative and operational support to the team and undertake ad hoc duties as assigned by the Reporting Officer.
