SingHealth Regional Health System (RHS) seeks a Director to support the Chief Operating Officer in RHS Operations and to implement population health initiatives across the eastern region. You will guide mid- and long-term planning, and establish governance for the Community of Care ecosystem.

The SingHealth Regional Health System (RHS) drives SingHealth's efforts in developing and strengthening partnerships across care providers in the eastern region - both private and public - and across care settings including tertiary, primary and community care. As the central funding conduit for all SingHealth Care Integration and Healthier SG initiatives, RHS provides overarching strategic direction to its programmes,supports service development, and monitors and evaluates key population health initiatives.
As Director, you will support the Chief Operating Officer of RHS Operations and clinical site leads, working alongside the SingHealth Office of Regional Health (SORH) to implement population health initiatives, guide mid and long-term strategic planning, and put in place the policy, planning and governance framework to support a future-ready health system through an integrated Community of Care (COC) ecosystem.
You will oversee and coordinate the SingHealth Healthier SG team's service delivery and resource deployment withinthe COC, while reviewing all services operating under the COC and providing advice and governance on operational issues.
In the area of operations and strategic planning, you will work with relevant internal and external stakeholders to formulate, implement and review strategies to deliver value-based care to the population in the eastern region. You will drive and coordinate policies that facilitate population enrolment, and engage process owners and business units to establish business insights that support value-based care and capitation pilot initiatives through data analytics.
For the implementation of Next Generation Primary Care, you will engage regional General Practitioners (GPs) and Primary Care Networks (PCNs) to adopt care protocols, programmes, IT systems, and care support and financing arrangements. You will plan and facilitate the regional resident enrolment and service journey, and implement integrated community care teams comprising primary care providers, community nurses, care coordinators and community partners to support residents in adhering to treatment and sustaining behavioural changes that improve their health outcomes.
On corporate oversight and governance, you will act as the primary liaison for RHS matters between sitesand the RHS group with the Ministry of Health (MOH) and Agency for Integrated Care (AIC), put in place a planning and governance framework to enable monitoring and accountability of key stakeholders, and establish a Risk Management framework to govern the programmes.
In the area of performance monitoring and programme evaluation, you will work closely with SingHealth RHS stakeholders to develop key performance indicators, review the progress and effectiveness of RHS initiatives, and recommend improvement plans. Together with Strategic Finance, you will evaluate the cost-effectiveness of right-siting efforts and drive the implementation of the national population health strategy in the capitation funding model to support whole-population enrolment within the eastern region. You will also provide leadership in data analytics to identify needs and areas for improvement.
As a partner engagement leader, you will build strong partnerships with internal and external stakeholders to improve health across all age groups, work with sites and institutions to establish system enablers aligned to the cluster's population health strategy,and cultivate strong partnerships with community partners and agencies including MOH, AIC and social and community organisations to drive programmes that promote healthy lifestyle choices.
You will also contribute to manpower development by working with the Group Director to develop training programmes that build a core team of staff well versed in population health, and grow a credible pool of staff with competencies and subject matter expertise in this domain.
