Job summary

Dementia Singapore Ltd is seeking a dedicated individual to support persons with dementia and their carers. You will conduct assessments, formulate individualized care plans, provide psychoeducation and coordinate with healthcare professionals to ensure holistic care.

The role includes documenting case records and contributing to programme evaluation. The ideal candidate has a degree or postgraduate diploma in social work, psychology or counselling, with 2 years of healthcare/community care

Qualifications

• Degree or postgraduate diploma in social work, psychology or counselling.
• 2 years healthcare/community care clinical experience preferred.
• Excellent oral, written and interpersonal communication.
• Ability to work occasional Saturdays and weekday evenings.
• Experience in dementia care is a plus.

Responsibilities

• Conduct case assessment and formulate Individualized Care Plan for Persons with Dementia and their carers.
• Provide intervention such as psychoeducation, supportive counselling and service linkages.
• Liaise and collaborate with multidisciplinary professionals and external service providers for holistic care management.
• Provide case recordings and relevant documentations.
• Participate in team meetings and case conferences as required.
• Assist in the preparation of data and statistics for programme evaluation and reporting purposes.
• Plan and facilitate Caregiver Support Group sessions.
• Plan, support and facilitate outreach events and talks.
• Any ad-hoc tasks as and when required.
