Job summary

Qualifications

• Registered with Singapore Nursing Board with valid practicing certification.
• Minimum of 4 years’ on-going nursing experience in Medical or Geriatric settings within the past 2 years.
• Experience in Oncology or Palliative Care is an advantage.
• Strong communication and interpersonal skills with all stakeholders.

Responsibilities

• Provide home based nursing care services requiring substantial and specialized nursing skills.
• Conduct Caregivers’ training onsite at client’s place of residence.
• Anticipate needs and risks and provide advice to mitigate them.
• Plan, develop, deliver and evaluate person-centered nursing care plans considering clients’ needs and preferences promptly.
• Conduct initial assessment and 6 monthly reviews in a timely manner.
• Execute care plans and interventions appropriately and timely.
• Practice infection control measures per St Luke’s Hospital policies.
• Manage patient workload through efficient planning and scheduling based on client’s needs.
• Document accurately after home visit within 1 workday.
• Comply with service guidelines and policies for safe care.
• Participate in Homecare Multidisciplinary Meetings and team huddles.
