Job summary

RECRUIT EXPRESS PTE LTD is seeking a Permanent Home Care Nurse to provide nursing support to patients at their homes in Singapore. You will conduct assessments, develop individualised care plans, and train caregivers as part of a multidisciplinary team.

Required SNB registration and at least 4 years of medical/geriatric nursing experience, with recent acute or community nursing; oncology/palliative care experience is a plus.

Qualifications

• Nursing care for home-based patients.
• Ability to create and adjust individualised care plans.
• Train and support patients’ caregivers.
• Experience with end-of-life care is an advantage.
