Job summary

J2B LIM & CO PTE. LTD. is seeking a Care Programs Coordinator to assist in planning and administering programs for both centre-based and home care, and to work closely with the Care Team to execute activities.

You will help group clients for suitable activities, coordinate volunteer groups and third-parties, and ensure activities cover Physical, Social and Cognitive areas while aligning with care plans.

Qualifications

• Assisting with planning of programs for centre-based and home care.
• Assisting with the development of suitable programs for centre-based and home care.
• Ensuring the rehabilitation activities are aligned with care plans.
