Job summary

RN Care Pte. Ltd. is seeking a dedicated Care Coordinator to support wellbeing monitoring through regular check-ins and resource referrals in Singapore.

You will coordinate health and social care administration with multiple stakeholders and maintain care plan documentation while safeguarding client data. The role emphasizes empowering clients toward healthier lifestyles, meeting service level targets, and escalating complex cases to senior coordinators when needed.

Qualifications

• Min GCE N Level and above.
• Min 4 years working experience in contact centre or eldercare.
• Required to work 12 hours rotating shift.

Responsibilities

• Assist and provide support in monitoring clients' wellbeing through regular check-ins and directing clients to resources.

• Maintain updated documentation of care plans.
• Encourage and empower clients towards healthy lifestyle changes.
• Escalate complex cases to senior coordinators when required.
• Ensure service standards, execute daily workflows, and maintain client data confidentiality.
• Achieve service levels and maintain low abandoned call rates through efficient call handling.
• Support community outreach and engagement as required.
• Additional duties may be assigned by your supervisor as required.

• Support community outreach and engagement efforts as required.
• Additional duties may be assigned by your supervisor as required.
