Job summary

Swift Search Global Pte Ltd seeks an ACP/PPC Coordinator to support residents and their families in discussing and documenting preferences for future care. You will work with certified ACP facilitators and healthcare professionals to coordinate discussions, maintain accurate records and follow up when care needs change.

The role emphasizes compassionate communication, meticulous documentation, and timely escalation of complex concerns to ensure residents’ values guide decision-making and family

Qualifications

• Degree in Social Work, Counselling, Psychology or Nursing.
• At least two years of experience in a healthcare setting.
• Prior National ACP Facilitation training.
• Knowledge of the National ACP IT system is compulsory.
• Proficiency in Microsoft Word, Excel and PowerPoint.
• Strong organisational and interpersonal skills, with the ability to work independently and respond thoughtfully to sensitive situations.

Responsibilities

• Coordinate ACP discussions with residents, their next of kin and certified ACP facilitators.
• Support residents in expressing their values, wishes and care preferences, and guide families through the ACP process.
• Liaise with doctors, nurses and other healthcare professionals on ACP matters.
• Record and update ACP discussions, decisions and documents accurately.
• Follow up on ACP plans and arrange reviews when a resident's health or care needs change.
• Escalate complex medical, psychosocial or family concerns to the appropriate professionals.
• Support ACP awareness sessions for residents, families and caregivers.
