MMARS is seeking a dedicated Supports Planner to provide case management services to clients, guiding them toward appropriate services and outcomes. You will collaborate with professionals to create individualized care plans and advocate for clients' needs.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Human Services, or related field (Master’s degree preferred).
• Proven experience in case management or a related field.

• Ability to work independently and as part of a team.

Responsibilities

• Conduct assessments to understand clients' needs and develop personalized case management plans.
• Coordinate with service providers and community resources to ensure access to necessary services.
• Monitor clients’ progress and adjust plans to meet goals; provide emotional support and advocacy.
• Maintain detailed case notes and documentation per policy and regulations.
