You will perform assessments, coordinate resources, monitor progress, document cases, and advocate for clients with empathy and integrity, while enjoying a supportive, growth‑oriented workplace and remote work options.

Qualifications

• Bachelor’s degree required or equivalent in a related field.
• Experience with case management or related services.
• Strong communication and interpersonal abilities.
• Ability to work independently and as part of a team.

Responsibilities

• Conduct assessments to understand client needs and develop personalized case plans.
• Coordinate with service providers and community resources to ensure access to services.
• Monitor progress and adjust plans to meet client goals.
• Provide emotional support and advocacy for clients with respectful communication.
• Maintain thorough case notes and documentation per policies and regulations.
