Job summary

MMARS is seeking a Coordinator of Community Services (CCS) to provide case management support, coordinate community resources, and help clients reach their goals. You’ll work remotely, collaborate with professionals, and ensure timely follow-up while upholding client-centered care.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Human Services, or related field.
• Proven experience in case management or related field.
• Strong communication, organizational, and interpersonal skills.
• Ability to work independently and in a team.
• Client-centered, compassionate approach to service delivery.

Responsibilities

• Conduct assessments to understand client needs and develop personalized case management plans.
• Coordinate with service providers and community resources for necessary services.
• Monitor progress and adjust plans to meet goals.
• Provide emotional support and advocacy in a timely, respectful manner.
• Maintain detailed case notes and documentation per policy and regulations.
