MMARS is seeking a Coordinator of Community Services (CCS) to support clients through case management, coordinating services and resources. You will collaborate with professionals to craft individualized care plans and ensure timely follow-up, delivering compassionate support.

The role emphasizes client-centered service, strong communication, and the ability to work independently or as part of a team within a flexible, remote work environment.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Human Services, or related field (Master’s preferred).

Responsibilities

• Conduct assessments to understand clients’ needs and develop personalized case management plans.
• Coordinate with service providers and community resources to ensure access to necessary services.
• Monitor clients’ progress and adjust plans to meet needs and goals.
• Provide emotional support and advocacy for clients with timely, respectful service.
• Maintain detailed case notes and documentation per policies and regulations.
