Job summary

Comprehensive Rehab Consultants (CRC) is hiring an LPN/LVN Care Coordinator who will work fully remote delivering Chronic Care Management services, helping elderly patients thrive.

Responsibilities include post-discharge outreach, care planning, and care coordination using EMR/Care Management tools, with emphasis on patient engagement and documentation. This is a full-time role with competitive pay, PTO, and health insurance options.

Qualifications

• Active LPN/LVN or equivalent license.
• 2+ years of clinical experience with geriatric patients in post-acute settings.
• 1+ years of 100% remote telephonic patient interaction.

Responsibilities

• Clinical Assessment & Planning: Review EMR records, assess risks, and customize care plans.
• Care Transition & Monitoring: Follow post-discharge patients and reassess readmission risk.
• Patient Engagement & Education: Use motivational interviewing for self-management and education.
• Care Coordination: Communicate findings and goals with providers, patients, and caregivers.
• Documentation & Compliance: Log care plans and outreach in care management software.
• Collaboration: Work with providers and care team to ensure quality care.
