Job summary

Georgia Real Care, LLC seeks a dedicated Nurse Practitioner to deliver advanced in-home wound care for chronic and complex wounds. You will assess patients, develop treatment plans, and collaborate with healthcare teams in-home or potentially in an office setting.

The role emphasizes patient advocacy, high-quality care, and coordination with physicians to ensure comprehensive treatment and documentation for compliance.

Qualifications

• Wound care experience is highly desirable.
• Independent Medicare Part B credentials are strongly preferred.
• Ability to manage medical records efficiently while ensuring patient confidentiality.
• Excellent communication skills for effective patient interaction and collaboration with healthcare teams.
• Strong organizational skills to manage multiple patient cases simultaneously.

Responsibilities

• Conduct comprehensive wound examinations and health assessments for patients upon referral.
• Debride and graft chronic wounds that have not healed with at least 4 weeks of conservative treatment.
• Collaborate with physicians and other healthcare providers to ensure coordinated care.
• Maintain accurate medical records and documentation in compliance with healthcare regulations.
