Job summary

Uprise Health is seeking an EAP Clinical Case Specialist to provide comprehensive clinical assessments and solution-focused guidance from a remote home office. You will coordinate with medical staff, arrange referrals, and handle inquiries while maintaining high-quality documentation.

This role requires licensure (LCSW/LPC/LMSW/LMFT) and 3–5 years of clinical experience. Work hours align to Pacific Time (8am–5pm PST).

Qualifications

  • Master’s degree in behavioral health is required.
  • Clinical licensure (LCSW, LPC, LMSW, or LMFT) is required.
  • EAP and call center experience is required.
  • 3–5 years of experience in a clinical setting preferred.
  • Excellent written and verbal communication skills.
  • Ability to work Pacific hours (8am–5pm PST).
  • Experience using MS Office, SharePoint, and collaboration tools.
  • Bi-lingual Spanish/English is a plus.

Responsibilities

  • Conduct comprehensive clinical assessments and provide solution-focused guidance.
  • Coordinate with physician advisors and discuss cases with medical staff when needed.
  • Arrange discharge planning with facilities and health providers.
  • Coordinate referrals to physical health providers and social services as needed.
  • Record and resolve member and provider inquiries per policy; handle first-level appeals.
  • Triage crisis calls to ensure safety and appropriate care.
  • Manage EAP service calls as needed.
  • Maintain accurate documentation in the system.
  • Perform administrative duties as assigned.

EAP Clinical Case Specialist will play a key role in the success and continued growth of Claremont/Uprise Health by providing top notch clinical skills and customer service. Our Clinical Case Specialists are empowered to conduct a comprehensive clinical assessment and provide solution-focused guidance, advocacy and support on every call.

This position is required to work Pacific Time Zone Hours from 8 am to 5 pm.

  • EAP clinical services experience, including clinical call center operations, case management, crisis response, and employer-based programs.
  • Assess clinical information provided and apply medical necessity criteria.
  • Coordinate behavioral health physician advisor activities as well as consult on cases with medical complications.
  • Coordinate discharge planning needs with facility, health services providers, members and family.
  • Provide care coordination with and facilitate referrals to physical health providers, social services and disability services as needed.
  • Record and resolve member and provider inquiries and complaints in accordance with company policies and procedures for first levels of appeals.
  • Triage calls from members in crisis and assist to ensure safety and access to appropriate care.
  • Handle and triage Employee Assistance Program service calls as needed.
  • Maintain accurate and complete documentation in system.
  • Perform administrative tasks and other duties as assigned
  • Master’s degree in behavioral health field required
  • Clinical licensure required (LCSW, LPC, LMSW, or LMFT)
  • Ability to work pacific hours (8am -5pm PST)
  • Excellent written and verbal communication skills
  • 3-5 years of experience in a Clinical setting preferred
  • EAP and call center experience required

While performing the duties of this job, the employee regularly works in a home remote office near office equipment (telephone, computer). Verbal and written communication, telephone usage, filing, sitting, typing, reading, and carrying are required to perform the essential functions of this job.

The compensation range is based on the candidates experience and/or level of education or certifications.

$100,000 - $100,000 USD
