Job summary

Responsibilities

• Conduct assessments of needs, strengths and challenges of individuals and families in the community through comprehensive interviews.
• Develop and implement individualized service plans in collaboration with clients.
• Provide counseling, emotional support and crisis intervention to individuals and families.
• Advocate clients within the community, ensuring access to services, resources and opportunities that promote self-sufficiency and well-being.
• Collaborate with stakeholders, community organizations and government agencies to address systemic issues, develop partnerships and enhance service delivery.
• Conduct outreach activities and educational workshops to raise social awareness of social issues.
• Maintain accurate and up-to-date records of client interactions, assessments, service plans, progress notes and referrals in accordance with agency policies and legal requirements.
