Job summary

San Lorenzo Ruiz Charity (SJDMC) Inc. in Bulacan is seeking a compassionate Community Registered Social Worker to join our Civic & Social Organization team. You will conduct assessments, develop care plans, provide counseling, and connect clients with local resources to improve well-being.

Responsibilities include crisis intervention, advocacy, and coordinating with professionals to ensure clients receive appropriate services, with home visits and follow-up as needed.

Qualifications

• Bachelor's in Social Work and registration as a Social Worker are required.
• Strong communication and interpersonal skills for diverse populations.
• Empathy, compassion and ability to build rapport with clients.
• Excellent organizational and time-management abilities.
• Proficient in using computer applications for documentation and reporting.
• Resident of nearby areas such as SJDM, Bulacan or nearby.

Responsibilities

• Conduct comprehensive assessments to identify needs of individuals and families.
• Develop and implement personalized care plans with clients and monitor progress.
• Provide counseling, emotional support, and crisis intervention.
• Connect clients to community resources including healthcare, housing, employment, and financial programs.
• Advocate for clients' rights and coordinate with other professionals and agencies.
• Facilitate support groups and educational workshops.
• Conduct home visits and follow-up appointments to monitor well-being.
