Job summary

My Guardian is seeking a Clinical Care Coordinator - Registered Nurse to act as a frontline representative and coordinate comprehensive patient care across home settings. You will develop and monitor care plans, supervise staff, and ensure timely, compliant delivery of nursing services.

The role emphasizes wound care, medication administration, and collaboration with hospitals and multidisciplinary teams to address physical, mental, and psychosocial needs of residents and clients.

Qualifications

• Must hold valid RN license and be in good standing.
• Experience coordinating home-based or primary care is preferred.
• Ability to supervise care staff and provide clinical guidance.
• Strong clinical assessment and documentation skills.

Responsibilities

• Attend home assessments and create, implement and maintain client care plans, ensuring staff are briefed and understand client requirements
• Provide specialist nursing advice to non-nurse Case Managers, Service Coordinators and Home Care Workers, supervising staff
• Maintain up-to-date client plans and administrative records in line with policy
• Wound treatment and dressing changes
• Coordinate continuity of care by liaising with hospitals and other health professionals
• Liaise with and organise allied health as required by clients
• Report changes in a client’s health or wellbeing
• Administer treatments and medications with knowledge of their purpose and adverse effects
• Use assessment skills to identify needs and develop a care plan in a multi-disciplinary approach
• Serve as a resource and mentor to staff to maintain nursing standards

Clinical Care Coordinator - Registered Nurse

The Clinical Care Coordinator is a front-line position acting as a representative of My Guardian providing the services offered by the company. The Clinical Care Coordinator duties are not limited to those listed as the position has a wide scope of duties at times dependent on the needs of the company.

Key responsibilities

• Attend home assessments and create, implement and maintain client care plans, ensuring that staff are briefed and understand client requirements
• Provide specialist nursing advice to the non-nurse Case Managers, Service Coordinators and Home Care Workers, motivating, leading and supervising staff
• Maintain up to date, clear and accurate client plans, records and meet administrative requirements in accordance with policy, procedure and funding guidelines
• Wound treatment/care and dressing changes
• Supervise the continuity of care by liaising with hospitals, other community services, doctors and allied health professionals
• Liaise with and organise allied health as required by clients
• Report any changes in a client's behaviour or health/wellbeing
• Controls, prepares, and administer treatments and medications with knowledge of purpose and adverse effects of the nursing care provided
• Utilize professional assessment skills to identify physical, mental, and psychosocial needs of residents, patients and develop a care plan to meet these needs in a multi-disciplinary approach
• Functions as a resource person and mentor to peer and subordinates to maintain professional nursing standards
