Job summary

Elevate Calls Inc. is seeking a compassionate Registered Nurse (RN) for a fully remote, full-time role.

You will engage with patients, guide them in managing chronic conditions, and coordinate care with providers to improve health outcomes. In this position, you’ll perform monthly outreach, develop personalized care plans, monitor symptoms, and educate patients and caregivers on disease management and medication adherence.

Qualifications

• Valid RN license in the Philippines.
• Minimum of 1 year of clinical nursing experience (chronic care, case management, or primary care a plus).
• Strong knowledge of chronic conditions such as diabetes, COPD, hypertension, and heart failure.
• Excellent communication, critical thinking, and organizational skills.
• Comfort using EHR systems and virtual care tools.
• Ability to work independently while thriving in a collaborative team environment.

Responsibilities

• Connect & Care: Make monthly outreach calls to patients enrolled in our CCM program.
• Personalized Care Plans: Collaborate with patients and providers to design and update individualized care strategies.
• Early Intervention: Monitor symptoms, identify risks, and prevent complications.
• Coordinate Care: Work with primary care providers, specialists, and community resources.
• Patient Education: Equip patients and caregivers with disease management and medication adherence guidance.
• Document with Precision: Maintain accurate and timely patient records in our EHR system.
• Stay Compliant: Follow CMS guidelines for CCM while promoting preventive care and lifestyle improvements.

What You Bring
