Job summary

Ascend Therapy and Wellness Center is seeking a licensed Physical Therapist to administer physical therapy treatments and wellness services including Pilates and strength & conditioning. Responsibilities

• Performs comprehensive patient assessments and formulates evidence-based treatment plans in coordination with Physician and rehab team
• Carries out physical therapy interventions following Physician’s prescription using various modalities
• Re-evaluates rehabilitation status and adjusts plan of care with physician
• Plans discharge and provides home instructions, follow-up or referral
• Maintains accurate and timely clinical documentation per standards
• Collaborates with healthcare professionals to coordinate patient care
• Sets up treatment area and ensures equipment and workspace are ready for next patient
• Designs and leads Pilates sessions (mat/equipment) integrated into physiotherapy plans
• Designs safe strength & conditioning programs and guides clients through workouts
• Conducts fitness assessments and educates on body mechanics and nutrition basics
• Organizes continuing education, attends staff and patient case conferences, and supports quality improvement
