Ascend Therapy and Wellness Center is seeking a licensed Physical Therapist to administer physical therapy treatments and wellness services including Pilates and strength & conditioning. The role involves documenting patient progress, coordinating with physicians, and ensuring proper charging for procedures.

The Physical Therapist is responsible for administering physical therapy treatment to patients and handling wellness services (Pilates and/or strength & conditioning) to clients. He/she implements physical therapy as prescribed by the Physician. He/she also accurately documents the patient’s treatment and progress in all required forms in the patient chart in a timely manner. He/she ensures that charging of patient procedures is aligned with actual treatment sessions.
Performs comprehensive patient assessments and formulate evidence-based treatment plans to determine the plan of care in coordination with the Physician and other members of the rehabilitation team
Carries out a physical therapy intervention following the Physician’s prescription using any combination of treatment modalities
Re-evaluates patient rehabilitation status and modifies or discontinues plan of care in coordination with the patient’s Physician
Plans for discharge of patient in consultation with appropriate disciplines, taking into consideration the level of goal attainment, and provides appropriate home instructions, follow-up or referral
Maintain accurate and timely clinical documentation in accordance with legal and professional standards.
Collaborate with healthcare professionals across disciplines to coordinate patient care.
Sets up treatment area and performs aftercare for each therapy session ensuring machines and equipment used are kept in their proper places and treatment area is clean and ready for next patient
Design and lead clinical and fitness-based Pilates sessions (mat and/or equipment-based)
Integrate Pilates into physiotherapy plans to support rehabilitation, core stability, posture correction and injury prevention
Modify exercises based on individual patient needs, pathology, or physical limitations.
Monitor group and individual sessions to ensure safe technique and optimal results.
Conduct fitness assessments to design safe and effective individualized personal training programs.
Guide clients through strength training, functional movement, flexibility, and cardio conditioning.
Support clients in achieving wellness goals including injury prevention, post-rehab strengthening, or general fitness.
Educate clients on body mechanics, nutrition basics (as applicable), and healthy lifestyle practices.
Organizes continuing education trainings, community activities and charity physical therapy services as needed
Attends to monthly staff conferences and patient case conferences
Submits at least one (1) continuous quality improvement project yearly
Educates and provides consultation to patients regarding the purposes and benefits of physical therapy, the roles of the physical therapist and the planning/implementation of physical therapy sessions
Performs other duties that may be assigned by the immediate supervisor from time to time
