Chong Hua Hospital is looking to hire an experienced and highly motivated Patient Safety Officer who is responsible forcoordinating and communicating patient safety protocols among the hospital institution’s healthcare staff members, ensuring provision of quality care that is accessible, safe, effective, patient-centered, timely, efficient, equitable, and appropriate.This position implements and monitors all patient quality management activities for the hospital including patient care and patient relations, the hospital’s established standards & policies compliance, risk management and safety performance improvement, and infection control audit of the implementation of action plans established.

Job Duties & Responsibilities
Patient Safety Audit Management & Implementation
  • Maintains the effective and efficient execution of his/her tasks and responsibilities by ensuring consistent compliance with existing protocols, guidelines, processes, procedures, standards, etc. of the division, department, unit, and hospital.
  • Coordinates with the Patient Safety Manager in the development, implementation, and oversight of an effective Patient Safety Inspection and Audit that operates incompliance with established policies and procedures, and relevant regulatory requirements and industry standards.
  • Maintains current and appropriate knowledge on the existing patient safety policies and procedures developed and implemented in different areas of Chong Hua Hospital necessary to perform the internal audit function.
  • Conducts research on specific industry standards’ best practices and provides recommended action plans in its effective implementation across the organization.
  • Collaborates with Patient Safety officer and manager in the generation of the patient safety audit checklists based on the established hospital policies, procedures, and per unit established work instructions.
  • Assists the Patient Safety Officer in the development of the Patient Safety audit plan and work schedule.
  • Performs a standard closeout meeting with the audited staff and leaders every after an audit is conducted to discuss findings, raise concerns, and address urgent issues found during the audit.
  • Collates patient safety audit results and provides a copy to the Patient Safety Officer and Patient Safety Manager.
  • Discusses all patient safety audit results and issues or concerns encountered during the audit to the Patient Safety Manager for guidance and review.
  • Provides timely and accurate feedback on any major and minor non-conformity in the established standards, policies, and procedures to theconcerned department manager or leader.
  • Conductspatient safety audit report out to all affected departments and issue a corrective action request forms to all departments with non-compliance or observations that needs corrective action.
  • Provides recommendations increationof tools and documentations, e.g., work instructions, department manual, etc. that will validate compliance with the standards set.
  • Collaborateswith the management and staff on the monitoring of identified quality indicators as well as on the result of the data analysis, action plans and recommendations.
  • Collates raw quality indicator data from the assigned quality indicator(s) and provides feedback from time to time to the immediate superior for any issues encountered.
  • Monitors progress of the corrective action request form(s) provided to the concerned departments and review if corrective action(s) is/are effective.
Analysis of Quality Data
  • Responsible for generatingreportin graphical presentation based on the data collected; such data are classified as the Hospital’s priorities in terms of quality: (1) Required Clinical Monitors, (2) Required Administrative Monitors, (3) IPSG Monitors.
  • Analyzes all collated data in the assigned quality indicator(s) and provides recommendations if necessary.
  • Applies data sampling and analysis techniques to auditing activities, including where appropriate statistical methodologies.
  • Reviews and analyzes internal quality standards and compliance audit results and provides recommendations to improve compliance of the established standards, policies, and procedures.
  • Independently, and in a unique and complex situation, applies appropriate basic andadvanceproblem-solving tools and techniques.
  • Developsdata collection tools to meet project goals and enhance data quality.
  • Manages data standardization and data integrity by ensuring accuracy, completeness, consistency, validity and timeliness of all patient safety data.
  • May be assigned in a project that aims to improve specific areas working independently or with a cross-functional team. May be tasked to make necessary recommendations on performance of team members or cross-functional team member.

  • Excellent customer relation skills.
  • Demonstrated understanding of basic corporate office software, including Microsoft Office 365 applications.
  • Effectivecommunication skills. Proficiency in the Visayan dialect and/or Tagalog is required; ability to communicatewellin English is an advantage.
