• Experience with government healthcare or financial assistance programs is a plus.
• Experience handling sensitive patient information and documentation.

Responsibilities

• Assist patients through phone calls and other communication channels regarding government assistance programs.
• Guide patients through application processes, documentary requirements, and submission procedures.
• Educate patients on where and how to obtain required supporting documents.
• Explain approval timelines, availment frequency, program limitations, and other application requirements.
• Coordinate and follow up with patients regarding pending requirements and application status.
• Maintain accurate records and documentation of patient interactions and assistance provided.
• Liaise with government agencies and relevant stakeholders to facilitate processing and status updates.
• Provide professional, compassionate, and patient-centered support while handling sensitive concerns.
• Ensure compliance with documentation, reporting, and data privacy requirements.

Purpose of the Role

The Patient Care Navigator serves as a key support partner for patients by helping them understand and access available government financial assistance programs for their medication and treatment needs. This role is responsible for guiding patients through application processes, coordinating documentation requirements, providing timely updates, and ensuring a positive and compassionate patient experience while navigating complex healthcare assistance programs.
