Job summary

VISAYA in Bacolod invites a Care Coordinator Nurse to coordinate patient care, support chronic condition management, and ensure smooth transitions between healthcare settings. This onsite night-shift role emphasizes coordinated care delivery and patient advocacy.

Ideal candidates are BS Nursing graduates with an active PHRN license, 1–3 years hospital experience (tertiary hospital preferred), and strong communication and interpersonal skills.

Qualifications

• BS Nursing degree with active PHRN license.
• 1–3 years hospital/clinical experience, preferably in a tertiary hospital.
• Case Management or Transition of Care experience is an advantage.
• BPO experience is a plus, but not required.
• Non-licensed BS Nursing graduates with healthcare BPO experience may also apply.
• Strong communication and interpersonal skills.
• Willing to work onsite in Bacolod and on a night shift schedule.
