Visaya Knowledge Process Outsourcing Corporation is seeking a dedicated Care Coordinator/Case Manager with a strong nursing background to join our healthcare team in Bacolod. The role requires hospital/clinical experience and preferred case management or transition of care exposure, working onsite on a night shift.

Ideal candidates hold a BS Nursing, an active USRN license (US Compact State), and 1–3 years clinical experience.

Qualifications

• Graduate of BS Nursing.
• Active USRN license for any US Compact State.
• 1–3 years of hospital or tertiary clinical experience.
• Previous BPO experience is an advantage, not required.
• Preferably with experience in case management or transition of care.
• Willingness to work onsite in Bacolod and on a night shift schedule.
• Strong communication and interpersonal skills.

Responsibilities

• Manage and coordinate chronic care plans for patients with long‑term health conditions.
• Monitor and assess patient conditions to ensure proper care and smooth transitions between care settings.
• Collaborate with healthcare providers and case managers to facilitate seamless care transitions.
• Prepare and process clinical reports and documentation related to chronic care management.
• Follow protocols and apply clinical guidelines in patient care.
• Provide education and support to patients and their families regarding chronic condition management.
