Job summary

ALLIED SEARCH PTE. LTD. is seeking a Lead Social Worker to oversee patient-centered social work services within our healthcare setting.

You will assess the psychological, emotional, social, economic and physical impacts of a medical condition on patients and families. You will provide counselling, crisis intervention and emotional support, lead staff supervision, manage complex cases, and collaborate with healthcare professionals to develop holistic treatment plans.

Qualifications

• Minimum degree in Social Work or equivalent.
• At least 3 years of relevant leadership experience.

Responsibilities

• Assess the psychological, emotional, social, economic and physical impact of a medical condition on patients and families.
• Provide counselling, crisis intervention, and emotional support to patients and their families.
• Help patients and families access community resources such as financial assistance, housing, transportation and healthcare services.
• Provide leadership and professional supervision to social workers and casework staff.
• Manage complex cases through assessment, intervention, case planning and review.
• Provide counselling and crisis intervention to clients and families.
• Guide staff in applying evidence-based and strengths-based social work practices.
• Conduct case reviews and ensure appropriate risk assessment and safeguarding.
• Develop and maintain partnerships with community agencies and other stakeholders.
• Lead groupwork, community initiatives and outreach programmes where required.
• Collaborate with other healthcare professionals to develop treatment plans that address holistic needs.
• Maintain accurate and confidential records of interactions with patients and families.
