Job summary

Mission Hospice and Home Care seeks an Inpatient Hospice Nurse to provide end-of-life care on the Stanford Hospital campus. This per diem RN role requires availability for at least 4 shifts/month, 8:30am–5:00pm, with a focus on symptom management and pain control in collaboration with the Stanford team.

The position calls for an AA or BSN, California RN license, and experience in acute care, home health, or hospice. Bilingual Spanish is a plus, with competitive pay and generous benefits stated.

Qualifications

• AA or BSN from an accredited nursing school.
• Experience in acute care, home health or hospice preferred.
• Current California RN license; PHN cert desirable; HPNA cert preferred.

Responsibilities

• Perform initial and ongoing hospice eligibility assessment with Hospice MD and CMS guidance.
• Use the nursing process to assess, plan, implement and evaluate patient/family needs.
• Coordinate social work and spiritual support services for the patient/family.
• Coordinate plan of care and ensure care coordination with inpatient Stanford team.
• Maintain regular communication with the Stanford Hospital team and other members.
