Job Summary
The Company Nurse – YAKAP Program Coordinator is responsible for coordinating and facilitating the implementation of the PhilHealth YAKAP Program for both employees and patients/members who are eligible to avail of the program. The position assists patients throughout the YAKAP process, coordinates with healthcare providers and PhilHealth, facilitates consultations and preventive healthcare services, maintains accurate records, and ensures that patients receive appropriate and timely assistance in accessing their available benefits.
Duties and Responsibilities
- Manage and coordinate the day-to-day implementation of the PhilHealth YAKAP Program for eligible employees and patients/members.
- Assist patients and members with YAKAP registration, verification, enrollment, and other program requirements.
- Explain YAKAP benefits, covered services, eligibility requirements, and the process for availing healthcare services.
- Facilitate and coordinate YAKAP consultations, primary care services, laboratory procedures, and other covered preventive healthcare services.
- Coordinate with physicians, laboratories, clinics, hospitals, and other accredited healthcare providers to facilitate patient services.
- Assist patients in scheduling consultations and follow-up appointments with appropriate healthcare providers.
- Monitor patients' YAKAP availments and ensure that required services and follow-ups are properly documented.
- Conduct basic nursing assessments and provide appropriate health education and assistance to patients, as applicable.
- Assist in identifying patients who may require follow-up care, preventive services, or further medical evaluation and coordinate with the appropriate healthcare provider.
- Maintain accurate and updated patient/member records, YAKAP documentation, and healthcare utilization records.
- Ensure that patient information and medical records are handled with strict confidentiality and in accordance with applicable data privacy requirements.
- Coordinate with PhilHealth representatives and accredited YAKAP providers regarding eligibility, benefits, documentation, and patient concerns.
- Assist patients in resolving issues related to YAKAP registration, availment, scheduling, and other program-related concerns.
- Monitor pending requests, referrals, laboratory results, and follow-up requirements to ensure timely coordination and completion.
- Prepare and maintain reports on YAKAP registrations, consultations, patient visits, availments, and other program-related activities.
- Support the implementation of health education, disease prevention, and wellness activities under the YAKAP Program.
- Assist in organizing health screening, preventive care, and other healthcare activities for employees and eligible patients/members.
- Ensure compliance with PhilHealth YAKAP guidelines, company policies, healthcare protocols, and established documentation procedures.
- Coordinate with internal departments and external healthcare providers to ensure efficient delivery of YAKAP services.
- Address patient/member concerns professionally and elevate complex or unresolved concerns to the appropriate supervisor or healthcare provider.
- Perform other nursing, patient coordination, and YAKAP-related duties as assigned by management.
Qualifications
- Bachelor's Degree in Nursing.
- Must be a Registered Nurse (RN) with a valid PRC license.
- Experience as a Company Nurse, Clinic Nurse, Staff Nurse, Healthcare Coordinator, or similar role is preferred.
- Experience handling PhilHealth programs, YAKAP, primary care, patient coordination, or healthcare benefits is an advantage.
- Good knowledge of healthcare services, preventive care, and patient coordination.
- Excellent communication and interpersonal skills.
- Strong organizational and documentation skills.
- Ability to communicate healthcare information clearly to patients and members.
- Proficient in Microsoft Office and healthcare information systems.
- Able to handle confidential patient and employee information professionally.
- Compassionate, patient-oriented, detail-oriented, and able to work independently.
