Job summary

The role involves reviewing orders, managing therapies, counseling patients, and contributing to safety initiatives and rounds. Hospital experience is preferred and PRC license required.

Qualifications

• Candidate must hold BS Pharmacy or PharmD and be PRC-registered.
• Hospital/clinical pharmacy experience is preferred.
• Strong knowledge of pharmacotherapy, drug interactions, and safety.
• Excellent clinical judgment, communication, and problem-solving skills.

Responsibilities

• Review and evaluate medication orders for safety and appropriateness.
• Provide clinical pharmacy services and collaborate with physicians and nurses.
• Participate in medication therapy management and reconciliation.
• Counsel patients on proper use, dosing, and potential side effects.
• Support formulary management and antimicrobial stewardship initiatives.
• Participate in multidisciplinary rounds and care planning.
• Monitor and report adverse drug reactions and outcomes.
