Job summary

Inter-Medical Unified Systems, Inc. is seeking a Clinical Pharmacist to provide pharmaceutical care and clinical services in coordination with physicians, nurses, and other health professionals in a hospital setting.

The role involves reviewing orders, managing therapies, counseling patients, and contributing to safety initiatives and rounds. Hospital experience is preferred and PRC license required.

Qualifications

• Candidate must hold BS Pharmacy or PharmD and be PRC-registered.
• Hospital/clinical pharmacy experience is preferred.
• Strong knowledge of pharmacotherapy, drug interactions, and safety.
• Excellent clinical judgment, communication, and problem-solving skills.

Responsibilities

• Review and evaluate medication orders for safety and appropriateness.

About the role

Clinical Pharmacist position providing clinical pharmacy services and pharmaceutical care to patients in coordination with physicians, nurses, and other healthcare professionals.

Key responsibilities

• Review and evaluate medication orders for appropriateness, safety, dosage, interactions, and potential contraindications
• Provide clinical pharmacy services and pharmaceutical care to patients in coordination with physicians, nurses, and other healthcare professionals
• Participate in patient medication therapy management, including medication reconciliation and monitoring of therapeutic outcomes
• Provide drug information and evidence-based recommendations to physicians and other healthcare professionals
• Monitor patients for adverse drug reactions, medication-related problems, and therapeutic effectiveness
• Participate in multidisciplinary rounds and contribute to the development and optimization of patient-specific treatment plans
• Provide patient counseling on proper medication use, dosage, precautions, possible side effects, and adherence
• Support medication safety initiatives, formulary management, antimicrobial stewardship, and other hospital pharmacy programs

About you

• Bachelor of Science in Pharmacy (BS Pharmacy) or Doctor of Pharmacy (PharmD) degree
• Registered Pharmacist (RPh) with a valid PRC license
• Preferably with experience in hospital or clinical pharmacy practice
• Knowledgeable in pharmacotherapy, medication safety, drug interactions, and evidence-based medicine
• Strong clinical assessment, communication, and problem-solving skillsAbility to work effectively with physicians, nurses, and other healthcare professionals
• Detail-oriented, organized, and committed to patient safety and quality pharmaceutical care
• Willing to work in a hospital environment and follow assigned schedules or shifts
