Qualifications

• Bachelor's Degree in Social Work
• 8 hours of relevant training
• 2 years of relevant experience
• RA 1080 (Social Worker) eligibility
• Sixteen hours of relevant training on SWDI, 4Ps Kilos-Unlad Case Management, technical writing, program monitoring and evaluation, data management and analysis, and social welfare and protection
• At least three (3) years of work experience in 4Ps Bureau level areas: data management and analysis, MOE, 4Ps Social Case Management, SWDI, Post-Service delivery, and social welfare and protection

Responsibilities

• Serves as the Division's Focal Person for case management services and SWDI validation/monitoring.
• Facilitates SWDI administration and analysis.
• Leads analytics to support management decisions and program improvement.
• Oversees case management interventions for Pantawid beneficiaries and post-service delivery.
• Provides technical guidance to Regional Case Managers on case management and SWDI.
• Coordinates with Field Offices to monitor case management interventions and referrals.
• Reviews regional SWDI reports to support policy development and planning.
• Develops and updates policies, guidelines, and monitoring tools.
• Designs and maintains dashboards to strengthen case management.
• Acts as a resource on case management and SWDI during meetings and trainings.
• Establishes partnerships to mobilize resources for case management initiatives.
• Provides inputs on policy issuances and participates in inter-agency meetings.
• Prepares reports and documentary requirements for Division and NPMO.
• Performs other related duties as assigned.
